A clinical note in Klinira is never edited in place and never deleted; a correction is a new entry that supersedes the old one, and both remain readable — which is what the Private Healthcare Facilities and Services regulations require.

Why append-only matters more than it sounds

A record that can be quietly changed is not evidence of anything. When a note is amended in Klinira the original stays, marked as superseded, with the time and the author of both versions. Nobody has to remember to keep the old one — it is not possible to remove it.

Templates for real consultations

9 note templates ship with the system, including SOAP, chronic disease review, antenatal, paediatric, procedure, dental and remote consultation. Templates are a starting shape, not a cage — free text is always available, because a real consultation does not always fit the form.

375 primary-care diagnoses are built in with Bahasa Malaysia terms alongside the English, and 33 of them are flagged as notifiable so the doctor is told at the point of diagnosis rather than at the end of the month.

Alerts that are worth reading

Klinira carries 92 drug interaction pairs and 15 allergy cross-reactivity classes. Severe alerts block and demand a typed reason; moderate ones appear inline; minor ones appear once. Four of the cross-reactivity entries exist specifically to switch warnings off — shellfish allergy does not contraindicate iodinated contrast, and an alert on everything is an alert on nothing.

Dental and periodontal charting

Charting is append-only in the same way notes are, so the state of a tooth at a visit two years ago is still visible rather than overwritten by today's chart. Dental clinics tell us this is the difference between a chart and a drawing.

Common questions

Can a doctor delete a note they wrote by mistake?

No. They can supersede it with a corrected note. Both stay in the record, which protects the doctor as much as the patient.

Is there AI note drafting?

It is coming in version 1.1 and it needs an internet connection. No clinical workflow depends on it: when it is unavailable the button simply greys out and the doctor types the note as always.
