A medical certificate, a referral letter, a JPJ vocational licence examination, a food handler's certificate, an insurance or pre-employment medical. Every one of them leaves the clinic and is acted on by an employer, an insurer or a licensing authority, and every one is numbered, printed from the record and kept on a register.

A medical certificate is issued from the consultation that justifies it, so the dates, the patient and the doctor come off the record rather than out of somebody's memory. The days count both ends, which is the mistake that matters: the ninth to the tenth is two days, and the ninth to the ninth is one. A printed zero is an argument with an employer, on a document the patient cannot get corrected without coming back.

Whether the diagnosis appears on it is a decision the doctor takes and the patient agrees to, as two separate ticks rather than one. An MC is handed to an employer the patient has to keep working for, so a system that quietly left the diagnosis off would leave the doctor believing they had made a choice they never made.

A certificate already handed over is never edited, because the patient is holding the paper and the employer may have a copy. It is cancelled with a reason and a new one is issued — and the cancelled entry stays on the register with its number never reused, because a gap in a numbered series is the first thing an auditor asks about.

Four examinations done for somebody who is not the patient

JPJ vocational licences, food handlers, insurance medicals and pre-employment medicals. The whole document goes to a third party, so the patient's consent and the recipient's name are both required before anything prints — consent to disclose is consent to disclose to somebody, and a record that says the patient agreed without naming who is a record of nothing.

Readings taken ten minutes ago are carried in rather than typed again, an unanswered question prints as “not recorded” rather than as a blank, and the count of unanswered ones sits beside the Issue button. A register, and a list of what lapses next

Everything issued sits on one numbered register that prints, with cancelled entries struck through rather than removed. Beside it is what falls due in the next ninety days — and a food handler's certificate runs three years from the anti-typhoid injection, not from the examination, so that date is read back out of the immunisation record instead of being typed a second time.

The renewal list names the form and the day the certificate stops being current, and never why the patient was examined. A food handler's certificate is about somebody's job, and that list is read at a counter.

Common questions

Do you print the actual JPJ form?

No, and deliberately. JPJ L8A is a nine-page government document with its own crest, JPJ has revised it before, and only the certification page is submitted anyway. A facsimile would look official and go out of date silently. What prints is your own copy carrying every figure in the order JPJ asks for them, so transcribing onto the real form is a straight copy.

Is a pre-employment medical the same as medical surveillance?

No, and getting that wrong makes an employer non-compliant. A worker exposed to a chemical hazardous to health needs medical surveillance under the Occupational Safety and Health Act 1994, which only an Occupational Health Doctor registered with DOSH may do, on DOSH's forms, with records kept thirty years.
