Patient privacy notice template
This is a template a Malaysian clinic can adapt and display for its own patients under the Personal Data Protection Act 2010; it is a carefully drafted starting point, not legal advice, and your adviser should review it before you use it.
What a patient notice has to cover
- What personal data the clinic collects, and why
- Who it may be disclosed to — panels, insurers, laboratories, referral recipients
- Whether providing it is obligatory, and what happens if the patient declines
- How the patient may request access or correction, and who to contact
- How long records are kept
- That the notice is available in Bahasa Malaysia and English
Practical points clinics miss
Display it where patients register, not only in a folder behind the counter. Keep both language versions identical in substance. Name a real person or role as the contact point rather than a generic address, and make sure that person knows they are it.
